PADI Open Water Diver certification

The PADI Universal Referral for Open Water Diver Certification is a document that student divers present to their instructor. It states that the student diver has completed initial training with another dive training organization. The form asks the instructor to instruct the student in open water diving, to sign a statement and to send it to the original instructor, who should certifiy the diver.

You can also use the Universal referral form for certification. However, PADI requires you to complete the course within 12 months of your last training. This is usually a two- or three day course. You and your buddy must be able, at all times, to dive to a maximum depth not less than 60 feet/18 m.

Medical statement

You will need to fill out the Medical Statement for Universal Reference Form if you want to become a scuba diving instructor. This is an important document that will ensure you're as healthy as possible before you go diving. It's designed to ensure your safety while you're underwater, so make sure to answer every question honestly.

First, verify that your physician signs the forms. This is a requirement when you first start training as a scuba diver. If you've developed a new medical condition, a new medical release is necessary. A photocopy of your certification card is recommended.

Logbook

A medical statement is required before a diver can open-water dive. This will usually be completed before the certification course begins. A doctor must sign the release. Changes in medical conditions will require a medical release. PADI provides a medical release. The PADI Release of Liability/Assumption Of Risk/Non agency Acknowledgement For - General Training is the equivalent of the medical release.

The universal referral is different from that used in a diving logbook. The student will be able to list all open water dives completed and the knowledge development sections. This document also includes an instructor signed certification that the dives were completed. The student then takes this form back to his or her original instructor who is supposed to certify him or her as a certified diver.

The course requirements

Most well-known scuba certification agencies participate in the Universal Referral Program. This means that a diver can take the theory and pool part of his or her open water course with his or her local dive center but complete the open water check-out dives with a different agency. This can be done by a dive center that has a universal referral program like SSI or NAUI, NASDSS, YMCA, and other organizations.

PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. Additionally, the student diver must provide a medical release signed by his or her physician before diving. To be certified, the student diver must take a photo. Digital images are accepted.
