PADI Open Water Diver certification

A student diver should present the PADI Universal Referral form for Open Water Diver Certification (PDF) to a certified instructor. The form states that the diver received initial training at another diving school. It directs the dive instructor to complete open water dives for the student, sign a certification statement, and then send the form to their original instructor, who should certificate the diver.

Although you can use the Universal Referral Form to obtain the certification, PADI insists that you complete this course within twelve months from your last training. It is typically a two- to three-day course. During this time you will need to be able to dive together to a maximum depth 60 feet/18m.

Medical statement

To become a certified scuba diver, you will need to complete a Medical Statement for Universal Referral. This important document will help you ensure your health before you dive. This document is meant to help you stay safe while diving, so be sure to answer every question honestly.


First, check that your physician has signed the form. This is necessary when you begin training to be a scuba diver. If you have a new condition, a medical release will be required. You should also take a photocopy your certification card.

Logbook

A medical declaration is required before a diver may dive open water. This will usually be completed before the certification course begins. A doctor must sign the release. You will need a new release if your medical situation changes. PADI allows you to obtain a medical waiver. This medical release is equivalent in size to the PADI Liability Release/Assumption and Risk Form - General Training, or the EU version.

The universal referral forms are different from those used in a dive logbook. It will include information about the dives that the student has completed, as well as knowledge development sections. This document also includes an instructor signed certification that the dives were completed. The student takes the completed form to his or her instructor and asks for certification.

Course completion requirements

Most well-known scuba certification agencies participate in the Universal Referral Program. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This is possible with the help of a diving center that participates in a universal program such as SSI and NAUI or NASDS.

PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. A medical release must be signed by the student diver before he or she can dive. To accompany their certification card, the student diver must also submit a photograph. Digital images are acceptable.
