PADI Open Water Diver Certification
A student diver should present the PADI Universal Referral form for Open Water Diver Certification (PDF) to a certified instructor. The form states that the diver received initial training at another diving school. The form directs the instructor of diving to complete the student’s open-water dives and sign a declaration. This statement should be sent to the original instructor so that they can certify the diver.
PADI will require that you complete the course within 12months of your last training. This usually takes two to three days. You must be able and willing to dive with your buddy up to 60 feet/18 metres.

Medical statement

The PADI Release Of Liability/Assumption Risk/Non Agency Acknowledgement Form-General Training or the EU version is equivalent to the medical release.
The universal referral form differs from the one in a logbook. It will list the open water dives and knowledge-development sections the student has completed. This document also includes an instructor signed certification that the dives were completed. The student must then take this form to the instructor, who will certify him/her as a certified diver.

Course completion requirements
The Universal Referral Program includes most of the well-respected scuba certification agencies. This means that a diver could take the theory, pool and open water courses with the local dive center while completing the open water inspection dives with another agency. This can only be done in cooperation with a dive centre that is a member of a universal referral programme such as SSI (NAUI), NASDSS or YMCA.
PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. The student diver must also sign a medical waiver signed by their doctor before diving. A student diver must also attach a photo to his or her certificate card. Digital images can be accepted.
